Cosmetic Surgery Course (PSCS Fellowship Training Program)

To become a legitimate Cosmetic Surgeon, a doctor (after 9 years of studies ending high school) needs to first undergo a residency training in a surgical specialty (ie general surgery, ent, ophthalmology, ob-gyne, etc) for 4 to 5 years. 

     Upon completion, he/she further goes into the sub-specialty training in Cosmetic Surgery for 2 years. Here surgeons are trained with exposure to different Cosmetic Surgery cases in hospitals and clinics where actual procedures are done.

     The Philippine Society for Cosmetic Surgery offers a 2-year comprehensive centre-based Fellowship Training in Cosmetic Surgery at the Centuria Medical Makati.

     Training staff are composed of top-notchebd experienced Cosmetic Surgeons from the Philippines and Korea.

Injected Nose

Why you should avoid Filler Injections for nose lifting? 

A lot of patients are swayed by some practitioners’ marketing scheme about “liquid nose lifting” using fillers. The promise of non-surgical and less downtime are quite tempting (even if the price is the same or even more expensive compared to certain surgical noselift or rhinoplasty). 

Below are complications for nose filler injections:

1. Blindness (when materials get into blood vessels draining to the eyes)

2. Avatar-look and Witch-look (since fillers are semi-solid, they tend to slide down to the sides of the nose bridge to settle between the eyes, and down the Columella or below the tip)

3. Infections

4. Skin necrosis

5. Temporary effect (less than 2 years for absorbable fillers)

6. Skin nodulations or bumpiness (for non-absorbable or permanent fillers)

We reserve the use of fillers (absorbable) to correct minor defects of the nose such as small bumps or depressions.
Implant Rhinoplasty (Silicone or Goretex) and Cartilage Rhinoplasty (harvested from the ear) are still the best options for augmentation/enhancement of the nose. 

For wide nostrils, we recommend Alar trimming. And for tip correction, we advise tip plasty with Cartilage.

More and more women are inquiring about this procedure. When asked why they want to undergo such, their reasons vary. The most common response is they want to restore the tone of their vagina the way it was before they gave birth (by normal delivery).

Others say that they want to heighten sexual pleasure not only for their own benefit but also that of their partner. Some say that the aging process affects the feel and look of their private part

Vaginoplasty is a female, surgical, genital procedure that aims to tighten the vagina. It involves removing excess vaginal lining and repairing surrounding soft tissues and muscles.

The procedure may be done in women who have had one or more vaginal births. Passage of a baby in the birth canal stretches the vagina and surrounding tissues. After delivery, the pre-delivery diameter of the vaginal canal is never attained no matter how much Kegel exercise these women tried.

These women during sexual intercourse may feel loose and although the male partner may notice a change, he may not mention anything about it.

Feeling of looseness may also be experienced by other women who have not delivered a baby yet, but still may undergo vaginoplasty to increase friction during intercourse resulting in a higher level of sexual satisfaction.

The procedure lasts for 1 to 2 hours and may be done in the outpatient setting or in the hospital.

If done in the outpatient, anesthesia used is mild sedation and local anesthesia. Patient stays in the recovery room for 2 to 3 hours and may go home when fully awake and ambulatory.

If done in the hospital , and the patient chooses to be admitted, saddle block anesthesia is done with the option to sedate or not depending on the desire of the patient. Patient is monitored in the recovery room for 4 hours and transferred to a regular room thereafter. She may be discharged after 24 hours.

Risks associated with surgery include bleeding, pain and bleeding. Bleeding is addressed during surgery. Post-operatively, pain and infection are addressed with analgesics and antibiotics.

Patients are advised follow-up after 6 days, may return to work after 7 days and may have sexual intercourse after 4-6 weeks.

After surgery, the vaginal tightness is markedly improved. Patients attest that they felt the difference and so did their partner.

Other names: Gore-Tex, ePTFE, Expanded Polytetrafluoroethylene

PTFE is a synthetic polymer commonly used in nonstick pans (Teflon) and insulators of computer cables.

ePTFE, a form of PTFE, was co-invented by father and son Wilbert and Robert Gore (Gore-Tex) in 1969 when they accidentally expanded PTFE to create a porous and lightweight material containing 70% air.

Medically, ePTFE are used as an artificial substitute for replacing damaged blood vessels. This use negates the need to harvest vessels from another location.

Goretex or ePTFE use as nasal implants started only during the late 1990s (early 2000s in Asia and almost a decade later in the Philippines). Initially, they were in the form spongy sheets where cosmetic surgeons lay them on top of each other, stitch them together, then place over nose bridge area to provide the lift. Results then were not gratifying, until recently, when Goretex are produced in the form fit for nose lifting (augmentation). They are now shaped like those of silicone implants as a one piece implant (where minimal carving is required).

Advantages of Goretex over Silicone implant:

1. Less chance of having implant extrusion since it has a softer and lighter consistency.

2. More permanent because of its porous surface (tissue tend to adhere to it thus less displacement).

3. Less chance of implant rejection.

4. More Natural looking.

TYPES OF BREAST IMPLANTS 

> A. Saline-filled implants

> B. Silicone gel-filled smooth implants or Silicone gel-filled textured implants

> C. Polyurethane (PU) coated implants

Breast Implants

Smooth and Textured implants have elastomeric silicone shell (envelope). Studies have shown non-smooth shell (textured) implants decreases the chance of development of capsular contracture (a common long-term complication of breast implants). PU coated implants are also silicone filled and claims an even lower rate of  contracture. Only A and B are approved for use by the US FDA.

Based on shapes, the common ones are rounded. The newer versions are anatomic (tear-shaped) and carry a higher price.

Saline filled implants have more tendencies to cause cosmetic problems like rippling and wrinkling compared to silicone-filled implants (filled with viscous materials).

IMPLANT PLACEMENT

Implant placement varies with the volume of existing breast tissue and skin/fat layer thickness. Behind the breast (sub-glandular) for those with adequate breast tissue. Deeper under the muscle placement is recommended for very thin subjects.

Sub-glandular placement is the most preferred (if possible) as it places the implants in the most anatomical location resulting to better and natural look.

How Botulinum Toxin (BOTOX®) works in Hyperhidrosis
1. BOTOX® is injected in small portions using fine needles and spread-out as a grid on affected areas (underarms, hands, or feet).
